Parking/Transportation:

  • Free Parking in the visitor’s lot

Services:

  • Referral to PrEP prescriber (can often be seen same day)
  • Free testing for STI/HIV/Hepatitis C
  • Risk reduction counselling
  • Laboratory monitoring
  • On site Pharmacy support
  • Prescription assistance programs

What to Bring:

  • Proof of Income (i.e. 2 current pay stubs, current years taxes or W2 )
  • Proof of Residence. Must have your name on the bill (i.e. driver’s license, current utility bill such as, water, cable, electric or landline phone bill)
  • Proof of Insurance: provide insurance card if applicable

Financial Matters:

  • Payment is required upon receiving services. Services will be rescheduled if payment is not available at time of service.
  • Initial Visit, 3 months, 6 months, 9 months, and 12 months: $48
  • 30 day visit: free
  • Individuals can apply for pharmaceutical assistance if they are uninsured or under insured.

Languages:

Interpreter services are available for any language. Written material is available in English and Spanish
